To begin, go to "Set up" in the main Insight navigation bar. Then click on "Setup Areas" and "Intake Forms Setup". Now click "Add Form". At this time, you may name the form. Note that this title will only be seen by staff. The form header and caption will be visible to clients on the app and on any printouts. Select the client information you wish to include on the printout and whether you want clients to verify their contact information on the app. You may also determine how often your clients will be required to fill out the form. In the header, you may also include your business's logo. To start adding questions, create a new form section by clicking the "Add" icon. You can title this section, then click the question pickup. Here, you can type in the question and then select the question type. Choices include multiple choice, checkes, short or long answer, top-down, or rating. For multiple-choice and dropdown questions, you'll see the choices. Click "Add" to input your options. There is also an "Other" button which will allow the client to type in an answer. Finally, you may delete a choice with the "X". Check questions allow you to select a minimum and maximum number of answers. Rating questions allow clients to be given a scale from 1 up to 10.
